Submission Guidelines:  Complete coupon (there are 9 below – one for each week of the contest period).  Place inside of a ziplock bag with your Labels/Boxtops.  Turn these in to your homeroom teacher at the end of each week.  (This year, treats will not be given in the lunchroom for each 15 labels.  Rather, a treat for each participant will be given at the end of each nine-week contest period.  Coupons MUST be completed to qualify!!)
